Our index sets consist of 8, 6 or 4-chart dividers with Mylar colored tabs. Tabs feature popular titles plus 1- write on blank, erasable tab designed for your specific needs. All tabs are printed with the same title on both sides and reinforced with tear proof Mylar.

• These stock medical chart dividers include 100 sets of 4 divider sheets with pre-printed, 1/6th cut side tabs.
• Each sheet is 8-1/2" x 11" with a the mylar laminated tabs extending an additional 1/2" on the side.
• These divider sheets are made out of a lb. (90#) manila stock and tabs are printed in black ink on both sides. Each sheet has five, mylar reinforced, holes punched on the side and two holes punched on the top.
